To whom it may concern:
This letter is a request for an In-Person Admin per Se hearing in San Luis Obispo, California. I was arrested for an alcohol related offense.
I am requesting that the department's evidence be sent to me at the earliest opportunity.
I am also requesting that the department issue a stay on the suspension of my driver's license pending the hearing in this matter.
Also for my records, can you please send me a temporary driver's license or in the alternative a letter from you acknowledging the stay on the suspension of my license.

(NOTE! By typing and mailing this letter you are asking DMV for a hearing and for a new temporary license so you may continue to drive until the hearing.
The DMV is pretty good at getting these temporary licenses sent out, however, your request is often part of a very big stack of similar requests!
If you do not receive your new temporary driver's license, I advise you to call DMV at the above number several days prior to the end of your 30 day license to inquire if the DMV received your hearing request and if your new temporary license is on its way.
DO NOT DRIVE IF YOU DO NOT HAVE A VALID LICENSE.
